Agenda Item Request Form

This form, along with documentation that is to be considered/approved by Council and/or any support documentation, must be received by the City Clerk’s office no later than 12:00 noon on the Friday of the week prior to the regular Council meeting you have requested to be placed on.

Regular Council meetings are held on the second and fourth Wednesday of the month at 7:30 p.m. in the Council Chambers at the Moriarty Civic Center located at 202 Broadway Street (S). Agenda items shall be scheduled as soon as possible by the Mayor.
